Team Tobati 2026 Required Forms
Parents of Team Members must fill out the two forms below: 1. the travel authorization form which needs to be notarized as well as the 2. Team Tobati Liability Form. There will be a notary at our parent meeting on Jan. 6. Please arrive early if you wish to have your form notarized at KO. We will have the forms at the meeting as well.
Along with these two paper documents, parents will be emailed a Kingswood Oxford digital permission form to be filled out.
