All Team Members Need to Turn in the Following Items by January 20th:

1.      completed visa application form (attached below) with an additional xeroxed copy of the form (When the visa application asks for an address put "Macchi Brozzon family Tobati, Paraguay 516-262360.  For purpose of trip, put "Tourist".  When asking if you're going to work put "NO".)

2.    2 passport size photos.  1 affixed to each of the two visa application forms

3.      your actual passport 

4.      2 xeroxed copies of the page of your passport that has your photo and passport number on it.

5.      notarized parent consent form (attached below) along with 1 additional xeroxed copy of the form.

All items should be put into an envelope and turned in to Mrs. Dudzik, Mrs. Garcia, or Mr. Garcia by January 20th.
